Filled-in form vs agency printout: which one HR wants
HR asks for your "SSS", "PhilHealth", "Pag-IBIG" and "TIN", and people hand over the form they filled in, then get sent back. The reason is simple: a form you fill in is an application. The paper that proves you have a number is a record the agency issues after it processes the application. This page is for a new hire or a member getting ready to go to a branch. At the end you will know which paper is which and what to bring in one trip.
PhilForms fills the application forms. It never issues a government record, and it is not affiliated with SSS, PhilHealth, Pag-IBIG, BIR or any government agency. Only the agency can give you the record.
The difference
Agency · You fill in (application) · The agency issues (record); SSS · E-1 Personal Record · SS number. After an online application SSS emails an SS Number Slip, a Transaction Number Slip and the E-1/E-6 form (SSS: Become an SSS member); Pag-IBIG · Member's Data Form (MDF) · Registration Tracking Number (RTN), then the MID number two to three working days later on Virtual Pag-IBIG (Pag-IBIG membership page); PhilHealth · PMRF · After you submit the PMRF you "await Member Data Record (MDR) and PhilHealth ID Card" (PhilHealth registration); BIR · Form 1902 · A TIN. The TIN card is a separate request on BIR Form 1905 and needs a 1x1 photo, a government ID and personal appearance (BIR Form 1905)
So the filled E-1 or PMRF is not your SSS or PhilHealth "record". It is what you submit to get one. Ask HR which they want, because HR sets its own list. Most ask for the number and a copy of the agency's record if you have it.
One at a time
SSS. SSS says securing a number does not by itself make you a covered member: an employee becomes covered when the employer reports them and pays at least one contribution (SSS: Become an SSS member). The same page says a number tagged "Temporary" works for contributions only. To be eligible for benefits or loans it must be "Permanent", which needs your PSA birth certificate or other primary documents. If you lose the number you must not apply for another one; SSS says verification can be asked at the nearest office.
Pag-IBIG. If you registered online, the MDF instructions say to print it back to back on a single sheet, and a paper registration needs one copy and a photocopy of a valid ID (Pag-IBIG MDF). Details in Pag-IBIG membership.
PhilHealth. Your PIN is permanent and the form says to use it in every transaction (PMRF). The MDR is PhilHealth's own printout. See the PMRF guide.
BIR. The 1902 is filed by your employer. Do not get a second TIN: the form's own warning says holding more than one is criminally punishable (BIR Form 1902). See BIR Form 1902.
What to bring to the branch in one trip
- The filled application, printed and signed - One government ID, and a photocopy (Pag-IBIG: at least one valid ID photocopy; BIR 1902: an ID that shows name, address and birthdate) - Your PSA birth certificate, and marriage certificate if it applies - Any number you already have (SSS, TIN, PIN, MID): never apply twice - The agency's own record if you already hold one, such as your SS number slip, MDR or TIN card - Your employer's details, if the form asks for them
Mistakes
- Handing HR the application instead of the record, or the reverse. - Applying again because the record is lost. - A name that differs from your birth certificate on any of the four.
The first job pack fills the four applications from one set of details. The full order is in First job requirements.
Questions
Is my filled-in E-1 my SSS number?
No. The E-1 is the application. The SS number is issued by SSS, and after an online application SSS emails the SS Number Slip, the Transaction Number Slip and the E-1/E-6 form.
What is the MDR, and does PhilForms issue it?
The Member Data Record is a PhilHealth record. PhilForms does not issue it; PhilHealth does, after your PMRF is processed.
I have a Pag-IBIG MDF printout. Is that my MID?
The MDF is the form. Your MID comes from Pag-IBIG on Virtual Pag-IBIG after you verify with your RTN.
Can I give HR a photo of my filled form?
Ask HR. PhilForms cannot say what an employer accepts.
