Affidavit of One and the Same Person (Philippines)
A sworn statement that two or more spellings of a name (or birth dates) on your records belong to you, drafted for the notary. It explains a mismatch; it does not correct a civil-registry record — a clerical error in a PSA record is corrected through the local civil registrar (RA 9048, as amended by RA 10172).
You will need: Your full name as it should be; Age in years; Complete address; Each way your name; How the difference came about, in your own words; Why you need it; City where you sign.
Free. Fill it in, download the PDF, sign it — and bring it to a notary public where it says so. PhilForms does not notarize and this is not legal advice.
