Authorization Letter (Philippines)

A signed letter that lets someone act for you at an office — claim a PSA copy, a document, an ID or a benefit cheque — in English or Tagalog. Not notarized. Some offices ask for a notarized letter or a Special Power of Attorney instead: check with the office first.

You will need: The language of the letter; Date of the letter; City where you write it; Who it is for; Your full name; Your address; Your valid ID and its number; Full name of the person you authorize; Their relationship to you; Their address.

Free. Fill it in, download the PDF, sign it — and bring it to a notary public where it says so. PhilForms does not notarize and this is not legal advice.

Not affiliated with BIR, SSS, PhilHealth, Pag-IBIG or any government agency.
