Petition for Correction of Clerical Error in a Certificate of Live Birth (RA 9048) (Philippines)
The petition, in the words and order of RA 9048 Form No. 1.2, to correct a clerical or typographical error in a birth record — or the day or month of birth or sex when it is clearly a clerical error (RA 10172) — filed with the local civil registrar where the birth was recorded (or a Philippine consulate abroad), not a court. Sworn before the administering officer; bring the supporting documents. The registrar may hand you its own copy of the form: the entries are the same.
You will need: Your full name; Your complete address; Whose birth record it is; Complete name of the owner of the record, if it is not yours; Their relation to you; Date of birth; City or municipality of birth; Province of birth; The registry number the birth was recorded under; Each error, one per line: "item no.
Free. Fill it in, download the PDF, sign it — and bring it to a notary public where it says so. PhilForms does not notarize and this is not legal advice.
