Request Letter for BIR Form 2316 (Philippines)

A short, polite letter asking an employer (present or former) for your BIR Form 2316 — and, if you want, a certificate of employment or your final pay. An employer must give the 2316 by January 31 of the next year, or on the day of the last salary payment when employment ends earlier (RR 11-2018, sec. 2.83.1).

You will need: Date of the letter; The company; Who reads it; Your full name; Your position there; When you started; Your last day, if you have left; The year of the 2316; Anything else to ask for, one per line; Where to send it.

Free. Fill it in, download the PDF, sign it — and bring it to a notary public where it says so. PhilForms does not notarize and this is not legal advice.
