Pag-IBIG housing loan application (HLF068): form, documents, co-borrower
HQP-HLF-068, the Housing Loan Application, is the form a Pag-IBIG member fills in to borrow for a house, lot, condominium unit or construction. This page is for a member getting ready to apply. At the end you will know what the form asks, what Pag-IBIG says to prepare, and which of the other numbered Pag-IBIG forms you do not need for a plain housing loan.
PhilForms fills the form. You submit it to Pag-IBIG yourself, and Pag-IBIG decides the loan. PhilForms is not affiliated with Pag-IBIG or any government agency. Whether you qualify, your loan amount and your rate are Pag-IBIG's call: ask Pag-IBIG or a branch.
Who can apply
I could not open a Pag-IBIG page that states the membership and age rules in words, so this page does not state them. What the form itself shows: the applicant writes a Pag-IBIG MID number or RTN at the top, so you must already be registered (form text, V09 03/2024). If you are not, see Pag-IBIG membership first.
What Pag-IBIG says to prepare
On Virtual Pag-IBIG's Housing Loan Application page, the checklist is (Virtual Pag-IBIG: Housing Loan Application):
1. The loan application form, HQP-HLF-068, with your 1x1 ID photo. A co-borrower uses form HQP-HLF-069, a separate form. 2. Proof of income. Pag-IBIG has separate lists of valid documents for the locally employed, the self-employed and OFWs. 3. One valid ID with signature, photocopy or scan. 4. A selfie photo showing your ID.
What the HLF068 asks
Open HLF068. The printed sections, in order, are:
- Loan particulars: purpose (lot, house and lot or condominium, construction, home improvement, refinancing, parking slot and others), desired loan amount, desired re-pricing period, mode of payment, and whether you already have a housing application. - Collateral: the property's location, type, developer or registered title holder, title (TCT/OCT/CCT) and tax declaration numbers, land and floor area. - Borrower's data: name, birth date, civil status, addresses, contact details, SSS/GSIS number, employer, TIN, occupation. - Spouse's personal data. - Bank accounts, credit cards, real estate owned and outstanding loans. - Miscellaneous: yes/no questions about past or pending cases, past-due obligations, closed bank accounts and a medical history question, with details if yes. - References, and the seller's data.
Answer the yes/no questions accurately. The form is a signed statement.
The other forms people mix up
The numbers are close, so check what each really is. I read each from its own PDF.

If a bank, developer or agent tells you to attach one of these to a housing loan, ask Pag-IBIG whether it is really needed.
Mistakes that delay a loan
- Using the HEAL co-borrower form (868) where the housing loan co-borrower form (069) is needed. - A missing or unsigned 1x1 photo, ID or selfie. - Proof of income from the wrong list (employed vs self-employed vs OFW). - Leaving the yes/no questions unanswered. - Different names or numbers between the form, the ID and the title documents.
Questions
Where do I get the co-borrower form?
Virtual Pag-IBIG lists HQP-HLF-069 for co-borrowers on its housing loan application page. PhilForms does not have it in its catalogue, so download it from Pag-IBIG.
Is HLF868 the co-borrower form for my housing loan?
No. HLF-868 is the co-borrower application for a Home Equity Appreciation Loan, per its own title.
Do I need an SPA to apply?
The SPA in this set, HLF-037, is for a moratorium on an existing housing loan. For a housing loan application, ask Pag-IBIG what it accepts if someone else will submit for you.
Can I apply online?
Pag-IBIG's Virtual Pag-IBIG has a housing loan application page that asks for the form, proof of income, ID and selfie. Whether you can finish the loan online is decided there.
