Senior citizen ID and purchase booklet: where to get them (OSCA)
This page is for a Filipino turning 60, or a family member helping one, who needs the senior citizen ID or the purchase booklet, or has lost one. At the end you will know which office issues them, what they cost, where they are valid, and what the law says about using them.
PhilForms fills the Quezon City OSCA application here; for any other city or town, the form is your own OSCA's.
Who is a senior citizen
A senior citizen is any resident citizen of the Philippines at least sixty (60) years old (RA 9994, section 2(a)).
Which office issues the ID and the booklet
Every city and municipality has an Office for Senior Citizens Affairs (OSCA), headed by a senior citizen whom the mayor appoints (RA 9994, section 6). Among the OSCA's duties, the law lists keeping the list of senior citizens up to date and issuing the national individual identification card, free of charge, valid anywhere in the country (same section). Its implementing rules add the purchase booklet, issued the same way: free of charge and valid anywhere in the country (RA 9994 implementing rules).
So you apply at the OSCA of the city or town where you live — not at a national agency, and not at another city's OSCA. Each OSCA keeps its own application form and list of documents.

What the ID is used for
The law gives senior citizens a 20% discount and exemption from VAT, where it applies, on listed goods and services bought for the senior citizen's own use — medicines and essential medical supplies among them (RA 9994, section 4(a)). The senior pack on this site lists the other privileges with their sources: Turning 60. National programs for senior citizens are with the National Commission of Senior Citizens.
Using the ID and the booklet properly
The implementing rules name what counts as misuse when buying medicines: using several purchase booklets, using the discount for medicines and supplies not for the senior citizen's own use, and someone else using the senior citizen's ID without authority (RA 9994 implementing rules). If a family member buys medicine for you, ask your OSCA or the pharmacy what they need to see.
