A family trip to Europe: passports and Schengen visas: your forms
A family is travelling to a Schengen country and each person, children included, needs a passport and a visa application.
Forms in this pack: Passport for each person; Schengen visa application, one per person; Leave application, if you work in government.
Passport for each person: Every traveller, a baby included, needs their own passport. Book at the DFA site; minors 7 and below need no appointment.
Schengen visa application, one per person: Each family member files their own form; a parent signs for a child.
Leave application, if you work in government: CS Form 6 asks where a vacation leave is spent, abroad included.
Passport appointments are free and are made only at passport.gov.ph; DFA advises not to buy tickets until the passports are in your hands.
Apply at the consulate of the country of your main stay, at least 15 days and no more than 6 months before the trip; the page lists the documents and the fee for adults and children.
Not for: Travel to a country outside the Schengen area (each country has its own visa form), or a minor travelling without a parent (see the DSWD travel clearance pack).
