CGAF for an immigrant visa: The BI forms index lists a consolidated general application form for an immigrant visa. This blank is headed Consolidated General Application Form for Immigrant Visa.
CGAF for a non-immigrant visa, SWP, or provisional work permit: The index lists a CGAF for a non-immigrant visa, including a provisional work permit and a special work permit. This blank is headed for those three.
CGAF for a student visa or special study permit: The index lists a CGAF for a student visa and a special study permit. This blank is BI Form CGAF-003-Rev 2 for those two.
CGAF for a 9(g) visa under IATF Resolution No. 131-A: The index lists a CGAF for 9(g). This blank is the consolidated form for 9(g) issuance under IATF Resolution No. 131-A.
CGAF for the applicant's children: The index lists a CGAF for children. This blank is the consolidated form for information on the applicant's child or children.
Quota immigrant visa application: The index lists a quota immigrant visa. This blank is headed Application Form for Quota Immigrant Visa.
Special visa for employment generation: The index lists the special visa for employment generation. This blank is headed Application Form for Special Visa for Employment Generation (SVEG).
An extension of a temporary visitor's visa asks for the Consolidated General Application Form for that extension, the original passport, and, if someone else files, a BI accreditation ID or a notarized special power of attorney. Philippine civil-registry documents in that packet must be PSA originals. The extension form itself is not a step here.
