Bid for a government contract: your forms

Reviewed by , founder of PhilForms.

Your business wants to bid for a government contract and needs the papers GPPB and PhilGEPS name.

Forms in this pack: Omnibus Sworn Statement; Bid Securing Declaration; Bid Form; Price Schedule, goods from within the Philippines; Performance Securing Declaration; Contract Form.

Omnibus Sworn Statement: Listed among the documents in the first bid envelope (bidding documents for goods, clause 12.1).

Bid Securing Declaration: One form of bid security from GPPB's standard forms; the first envelope needs a bid security in the prescribed form and amount.

Bid Form: GPPB's standard Bid Form for goods and infrastructure projects.

Price Schedule, goods from within the Philippines: GPPB's standard price schedule. Goods offered from abroad have a separate schedule.

Performance Securing Declaration: GPPB's standard declaration for the performance security, once you are awarded.

Contract Form: GPPB's standard Contract Form.

The first bid envelope lists your PhilGEPS Platinum certificate, a Statement of Single Largest Completed Contract, an NFCC computation or committed line of credit, and a statement of all ongoing government and private contracts (Philippine Bidding Documents for goods, clause 12.1).

GPPB's standard forms list has no separate form for the two contract statements, so use the format in the bidding documents you were given.

PhilGEPS registers a supplier as Red by default. Platinum needs scanned, valid copies of your business registration, mayor's permit, audited financial statement stamped received by the BIR, a notarized PhilGEPS sworn statement, a tax clearance, and a PCAB license for contractors.

PhilGEPS now calls the sworn statement the PhilGEPS Sworn Declaration of Compliance, and tells bidders to keep Class A eligibility documents current.

If you are told yours is the bid to check, you have five calendar days to submit your eligibility documents and latest income and business tax returns (bidding documents for goods, clause 27.1).

Not for: Bids for consulting services use their own bidding documents. A bid under the older RA 9184 forms has its own list on GPPB's site.
