Household employer with a kasambahay: your forms
You already employ a registered kasambahay and pay their premiums every month.
Forms in this pack: Monthly PhilHealth premium; Report a helper who joins or leaves; Update your own details.
Monthly PhilHealth premium: One PhilHealth Payment Slip per helper, by the 25th of the following month; you may pay up to two years ahead.
Report a helper who joins or leaves: Within 30 days of a helper leaving, file the report form (HEUR2) with their date of separation; a new helper is reported the same way.
Update your own details: A new address or phone number is filed on the employer registration form (HEUR1) again.
SSS and Pag-IBIG contributions for your helper are paid every month too, through each agency's own payment channels — there is no form to fill for those here.
