Leaving to work abroad for the first time: your forms
Reviewed by Mar Angelo N. Revelo, founder of PhilForms.
You have a job offer abroad and no Philippine employer behind you, and you want your papers and your SSS, PhilHealth and Pag-IBIG numbers in order before you fly.
Forms in this pack: Passport with at least one year left; Make your DMW e-Registration; SSS number (E-1 Personal Record); PhilHealth number (PMRF); Pag-IBIG MID number.
Passport with at least one year left: DMW's direct-hire list asks for a passport valid not less than one year.
Make your DMW e-Registration: Your profile with the Department of Migrant Workers, needed to get hired abroad.
SSS number (E-1 Personal Record): Only if you have no SSS number yet. Coverage is compulsory for land-based workers abroad (RA 11199 sec. 9-B).
PhilHealth number (PMRF): Only if you have no PhilHealth number yet. Every Filipino is covered, and migrant workers are direct contributors (RA 11223).
Pag-IBIG MID number: Only if you have no Pag-IBIG MID yet. Coverage is mandatory for Filipinos employed by foreign-based employers (RA 9679 sec. 6).
A direct hire's list also names a work visa or permit, the contract verified by the labor office abroad, a medical certificate from a DOH-accredited clinic, the PEOS certificate and the PDOS certificate from OWWA. Ask DMW what your country adds.
OWWA membership comes with the processing of your contract, or you register yourself at the job site or online.
The law makes proof of insurance a requirement for your Overseas Employment Certificate.
Not for: Seafarers (see Seafarer between contracts) or workers already abroad (see Overseas Filipino worker).
Not affiliated with BIR, SSS, PhilHealth, Pag-IBIG or any government agency.
