Overseas Filipino worker (land-based): your forms

A Filipino working abroad on a land-based contract who keeps SSS, PhilHealth and Pag-IBIG membership going from overseas.

Forms in this pack: SSS registration (E-1); Update your SSS record (E-4); SSS contribution payment (OFW); PhilHealth member registration / update (PMRF); PhilHealth Premium Payment Slip (self-paying); Pag-IBIG membership registration; Pag-IBIG member's data form; Pag-IBIG MP2 savings enrollment; OWWA OFW information sheet.

SSS registration (E-1): Once, if you do not have an SSS number yet. OFW coverage is compulsory.

Update your SSS record (E-4): When your address, civil status or beneficiaries change.

SSS contribution payment (OFW): You pay both shares as a self-employed-type member. SSS has no paper form for this: you generate a Payment Reference Number (PRN) in My.SSS and pay with it.

PhilHealth member registration / update (PMRF): Register or update your record as an overseas Filipino member.

PhilHealth Premium Payment Slip (self-paying): You pay the year's premiums yourself. Generate your Statement of Premium Account (SPA) in the PhilHealth Member Portal first, then bring this slip with the SPA number to any payment channel.

Pag-IBIG membership registration: Once, to get your Pag-IBIG MID number.

Pag-IBIG member's data form: When your details change.

Pag-IBIG MP2 savings enrollment: Optional: open an MP2 account, Pag-IBIG's voluntary savings program, on top of your regular savings. You need your Pag-IBIG MID number and one valid ID.

OWWA OFW information sheet: OWWA's downloadable-forms page names this paper: the Overseas Filipino Workers Information Sheet (FM-OWWA 07-14.02). The SSS, PhilHealth and Pag-IBIG steps above do not enroll you in OWWA.

OFWs earning only from abroad do not file a Philippine income tax return (RA 11976).

SSS coverage of OFWs is compulsory, but paying SSS is no longer a condition for your OEC (Supreme Court, G.R. No. 248680, promulgated 5 November 2024).

OWWA's downloadable-forms page names one paper form: the Overseas Filipino Workers Information Sheet.

The online membership page asks for a passport number and an upload of the contract, work permit, payslip, or OEC.

Not for: Sea-based workers (their manning agency remits as employer) or OFWs who also run a business in the Philippines.

Not affiliated with BIR, SSS, PhilHealth, Pag-IBIG or any government agency.
