A family member with a disability: your forms
Reviewed by Mar Angelo N. Revelo, founder of PhilForms.
Someone in your family has a disability and you want the PWD ID, the discount and PhilHealth in place.
Forms in this pack: Apply for the PWD ID; PhilHealth record (PMRF).
Apply for the PWD ID: File at your city or municipal social welfare office, or the PDAO where there is one. It issues the ID; NCDA does not, except to soldiers with disabilities. NCDA's charter lists a 1x1 picture and a medical certificate.
PhilHealth record (PMRF): All PWDs are covered automatically and the national government pays the premium (RA 11228). Use this to register or update the record.
The ID, your passport, or the NCWDP transport ID gets at least 20% off and VAT exemption where it applies: hotels, restaurants, medicines, medical services, fares and funeral services.
The same law provides educational assistance for PWDs through scholarships, grants, financial aid and subsidies. Ask your school or the social welfare office what is open.
Not for: A store working out how to give the discount, or a disability claim from SSS or GSIS (those are benefits with their own forms).
Not affiliated with BIR, SSS, PhilHealth, Pag-IBIG or any government agency.
