Register your kasambahay (household helper): your forms
You have hired a kasambahay — a yaya, cook, driver for the family or other household helper — and need to register yourself as their employer and enrol them with SSS, PhilHealth and Pag-IBIG, in order.
Forms in this pack: Register yourself as a household employer; Enrol your kasambahay; Report your kasambahay as your employee; Your first monthly premium.
Register yourself as a household employer: One form registers you as an employer with SSS, PhilHealth and Pag-IBIG at once (the agencies call it HEUR1). Leave the three employer-number boxes blank — the agencies give you those numbers.
Enrol your kasambahay: If your helper has no SSS, PhilHealth or Pag-IBIG number yet: one unified form (KURF) gets all three. It is about your helper — fill it with them, with their own details; their numbers stay blank until issued.
Report your kasambahay as your employee: Says who works for you, since when and at what monthly pay (HEUR2). Hand it in together with your helper's form from step 2. If they later leave, file it again within 30 days.
Your first monthly premium: One payment slip per helper, by the 25th of the next month. If they earn under ₱5,000 a month you pay the whole premium; from ₱5,000 they share it.
As employer of the kasambahay you register yourself with, and enrol them in, SSS, PhilHealth and Pag-IBIG (IRR of RA 10361, Rule V Sec. 3).
SSS and Pag-IBIG contributions for your helper are paid every month too, through each agency's own payment channels (an SSS payment reference number, Pag-IBIG's payment centres) — there is no form to fill for those here.
Not for: Workers in your business (see Small office with 1–10 employees).
Not affiliated with BIR, SSS, PhilHealth, Pag-IBIG or any government agency.
