Apply for a scholarship or replace a school paper: your forms
Reviewed by Mar Angelo N. Revelo, founder of PhilForms.
You are a student applying for a government scholarship or grant, or you lost a school ID and were told to bring an affidavit.
Forms in this pack: Bagong Pilipinas Merit Scholarship application (Annex A); SIDA scholarship: application for SRA certification; Affidavit of loss (school ID or other paper).
Bagong Pilipinas Merit Scholarship application (Annex A): Applications go through the BPMSP online portal. CHED accepts this paper Annex A only from applicants in geographically isolated areas with connectivity problems, through the nearest DepEd or TESDA office. Everyone else can use it to prepare the answers.
SIDA scholarship: application for SRA certification: For sugarcane industry workers, small farmers and their children or dependents. The paper says to submit it to the Mill District Officer.
Affidavit of loss (school ID or other paper): Only if your school or an office asks for one when you replace a lost ID. Each school sets its own replacement steps.
A new Tulong Dunong applicant needs the TDP-TES application form, a certificate of enrolment or registration with the number of units, and a certificate of indigency signed by the Punong Barangay where you live.
Tulong Dunong is for a first undergraduate degree, with household gross income not exceeding ₱400,000, and not for someone already on TES or another national government scholarship.
BPMSP takes only these as proof of income: a BIR certificate of tax exemption or non-filing, a DSWD or C/MSWD certificate for 4Ps beneficiaries, the latest Form 1701 or 2316 of both parents or guardians, or a certified true copy of the latest contract for children of OFWs and seafarers.
BPMSP also needs a PSA birth certificate and a certified true copy of your report card (SF9 or Form 138).
The SIDA paper lists what goes with it: the birth certificate, your parents' or guardian's latest income tax return or a BIR certification, a 1x1 picture, and an employment certificate or endorsement from an industry head it names.
Not for: Private scholarships and a school's own grants have their own forms. The TDP-TES application form is not in this catalogue yet: UniFAST lists it, and your school or CHED regional office can tell you where to get it.
