A government office puts CS Form 212 and leave forms on its website
Applicants and employees fill official Civil Service Commission forms and agency PDFs on the office portal to print and sign.
The Personal Data Sheet prints with shifted layouts, wrong fonts and broken margins.
People get stuck on single boxes, such as two schools or two courses.
People are unsure what counts on the Work Experience Sheet: internships, unrelated jobs, or nothing.
Every government job applicant and employee fills in the Personal Data Sheet, officially called CS Form No. 212, Revised 2026. A fresh graduate submits it when applying to an agency, and a current employee fills it in when updating a 201 file. The Civil Service Commission guide says every entry must be filled in, with N/A written if a box does not apply, dates in dd/mm/yyyy format, and names of schools, agencies, and positions written in full without abbreviations. When people rebuild the form in another program or use fit-to-page settings, the layout shifts and the HR office receives pages that do not match the official template.
The Revised 2026 version adopted under Civil Service Commission Memorandum Circular No. 05, s. 2026 changes several items from the Revised 2025 form. Item 13 now asks for the PhilSys Card Number (PCN) instead of the PhilSys Number (PSN). Item 28 on work experience adds columns for monthly salary and for salary, job, or pay grade and step in the format 00-0. At the bottom of every page, the guide requires the employee’s wet signature, e-signature, or digital certificate along with the date.
For daily personnel work, rank-and-file employees and HR staff also handle CS Form No. 6, Revised 2020, the Application for Leave. The form’s printed instructions state that it must be accomplished at least in duplicate with documentary requirements. Filing windows and attachments depend on the leave type: vacation leave is filed 5 days in advance whenever possible, sick leave of more than 5 days or filed in advance requires a medical certificate, and leave of 30 calendar days or more needs a clearance from money, property, and work-related accountabilities.
Alongside Civil Service Commission forms, a government office uses its own internal PDF sheets, checklists, and vouchers. These can be turned into fillable forms too, so the whole set sits in one place.
The agency website address: The office lists its portal domain, such as a main site or subdomain, when creating the embed site key so the forms only run on authorized pages.
The office’s own PDF files: HR or administrative staff upload internal PDF forms, up to 25 MB and 30 pages each, to turn them into fillable forms for the organization.
Government IDs and PhilSys Card Number: Applicants and employees need their UMID, Pag-IBIG, PhilHealth, TIN, agency employee number, and the PhilSys Card Number (PCN) for item 13 of CS Form No. 212.
Unfiltered passport-sized digital photo: Item 42 of the Personal Data Sheet requires an unfiltered picture measuring 4.5 cm x 3.5 cm taken within the last six months.
Eligibility and work experience records: Filer entries require exact exam ratings, dates, full school and agency names, monthly salaries, and salary grades in the 00-0 format.
Medical certificates or leave clearances: Employees filing CS Form No. 6 attach required proofs such as a medical certificate for sick leave over 5 days or an updated Solo Parent ID card.
1. Create a site key. The office staff goes to Account and then Embed to create a public site key that starts with pk_embed_. They list the exact website addresses where the forms will live, and can cover every subdomain one level deep. For a portal where employees or applicants sign in, the office keeps the key on its default server-mint-only setting and creates each session on its own server with its secret key. On the Government or Pro plan, embedded forms show no badge and nothing in the embed names PhilForms.
2. Embed CSC forms on portal. The web team pastes the embed script onto the agency page to show CS Form No. 212, Revised 2026 and CS Form No. 6, Revised 2020. Because the values are typed directly onto the official form template and sealed into a PDF, the page layout does not shift as it does when rebuilt in another program. The office can set the view to auto so visitors on phones get one question at a time while desktop users see the fields and live form side by side. The staff can also pass custom labels, placeholders, and an assistant FAQ with the office’s own answers on notarization, paper size, and copy counts.
3. Prefill and require core fields. When a signed-in employee opens CS Form No. 6 or updates a 201 file, the portal can prefill known details or lock fields the visitor should not change. The office can mount the form with the core requirement turned on so no PDF goes out while required fields like name or address are blank, while agency numbers a person may not have yet never block the fill. In Account and Embed, the chart icon on the site key shows how many visitors opened the form, started typing, got the PDF, or hit an error over the last 30 days, plus the fields they most often get wrong.
4. Upload the office’s own PDFs. For internal checklists or applications, staff upload the office’s PDF in Your forms to get a draft of detected fields, boxes, and types. They fix any field names, set rules like required fields or number patterns, and group tick boxes that answer a single question into one choice. Before the form can go live, the system requires the staff to preview a filled test copy and check every field on the page. Once published, the original PDF layout stays as the office designed it.
5. Share with the organization. The administrator shares each published custom form with the office’s organization account. Organization members can fill the form and scan paper copies into it, while owners and admins can edit and publish updates. For field offices or halls without a stable internet connection, members can pin forms in Offline mode—up to 25 forms on Pro and 200 on the Government plan—save completed entries to the outbox while acting as the organization, and let the server make the sealed PDFs once the device is back online.
6. Add the form button. To put an uploaded custom form on the agency website, the web team adds the PhilForms button with the custom form ID and the site key. Visitors click the button to see a simple field list on the page and download the filled PDF without ever seeing the form’s box layout data. If someone still submits a form filled by hand on paper, office staff can photograph or scan the pages in order and use the scan tool on the Fill page to read the handwritten values back into the form fields.
Writing abbreviations or leaving boxes blank. The Civil Service Commission guide for CS Form No. 212 requires every entry to have a value, with N/A written when an item does not apply. Names of schools, agencies, positions, and training providers must be written in full, such as writing Civil Service Commission rather than CSC.
Entering the PSN instead of the PCN. On CS Form No. 212, Revised 2026, item 13 asks for the PhilSys Card Number (PCN). Applicants who copy the PhilSys Number (PSN) used on the Revised 2025 version put the wrong number in the box.
Counting calendar days on CS Form 6. In item 6.C of CS Form No. 6, employees must count working days rather than calendar days and list the inclusive dates. Employees should also leave Part 7.A blank when submitting, because the authorized HR officer fills and signs the certification of leave credits.
Using the embed script for your own PDF. The full embed filler does not take a form the office uploaded itself. To put an office’s own uploaded PDF on a web page, the site must use the form button widget instead.
The visitor or employee gets a sealed PDF of CS Form No. 212, Revised 2026, CS Form No. 6, Revised 2020, or the office’s own custom form right from the agency website. If the office sets a webhook address on its site key, its server receives the document ID and the values on every fill. If its page listens to the filled event, it can forward the finished PDF to its own system. If a person spots a typo after making a PDF, refilling the same form for the same person with the first document’s ID within 7 days and fewer than 5 edits costs zero credits. Downloading the exact same values again in the same calendar month is also free.
PhilForms fills the PDF; it never files, notarizes, or approves anything. On CS Form No. 212, the applicant or employee must check with the agency HR office whether to print on 8x14 inch long paper or A4, attach an unfiltered 4.5 cm x 3.5 cm passport photo taken within the last six months, sign and date the bottom of every page and inside the box in item 42, and bring a government ID to the person administering the oath if the agency requires notarization. On CS Form No. 6, the employee prints at least in duplicate, attaches required documents such as a medical certificate or Solo Parent ID, signs item 6.D, and submits the paper to the HR officer for the leave credit certification in Part 7.A and the supervisor’s recommendation.
This setup does not decide which past jobs, internships, or OJT an applicant should list on the Work Experience Sheet; the Civil Service Commission guide states only that experience relevant to the vacant position goes on that sheet, and applicants must ask the hiring HR office what it counts. It also does not replace missing eligibility certificates, recover unreadable PhilSys numbers, set an agency’s notarization fees or copy counts, or change how many internal vouchers and proofs an office’s own accounting rules require.
Which version of CS Form 212 should our office and applicants use?
The Civil Service Commission adopted CS Form No. 212, Revised 2026 through Memorandum Circular No. 05, s. 2026, which updates item 13 to the PhilSys Card Number (PCN) and adds monthly salary and salary/job/pay grade and step (format 00-0) to item 28. The guide does not state a transition period, so ask the agency’s HR office which version it accepts today.
Should CS Form 212 be printed on legal (8x14) or A4 paper?
The Civil Service Commission forms page carries a note beside the Revised 2025 listing to specify 8x14 inch paper when printing, while the Revised 2026 guide gives no paper size and the PDS file on PhilForms is A4. Applicants and staff should confirm the required paper size with their HR office and avoid using ’fit to page’ when printing.
Does the Personal Data Sheet have to be notarized?
Page 4 of CS Form No. 212 ends with a ’SUBSCRIBED AND SWORN to before me’ block for the person administering the oath and the government ID shown. However, the Civil Service Commission guide does not state whether or when notarization is required, so the agency’s HR office sets the rule on notarization and how many copies to submit.
What goes into the Work Experience Sheet for a fresh graduate?
The Civil Service Commission guide says the Work Experience Sheet is submitted with the PDS when applying for a vacant position, only for experience relevant to that position. If a fresh graduate has no relevant experience, they write N/A and ask the hiring agency whether it wants internships or OJT listed.
Who fills out the leave credits table in Part 7.A of CS Form No. 6?
The employee leaves Part 7 blank and only completes items 1 to 6. The authorized officer in HR fills and signs Part 7.A to certify total earned vacation and sick leave, less the current application, and the remaining balance.
